Return Policy:

All sales are final. However, we realize that it is sometimes necessary for our customers to return product. All returns must be authorized by Covidien Customer Service 1-800-722-8772. Authorization must take place prior to any merchandise being returned. Only products purchased directly from Covidien will be accepted for return. Returns without Return Authorization Will Not receive credit and will not be returned.

Return Authorization

Covidien representatives and managers are not authorized to remove any product from your hospital and cannot authorize returns. Request for merchandise return authorization must include:

  • Reason for return
  • Item number/Covidien product code number
  • Customer purchase order number
  • Customer debit number
  • Quantity
  • Lot number

Customer service will assign a return authorization number. The customer must include the merchandise return authorization number on all paperwork for the return. The return authorization must be returned in one shipment within thirty (30) days of issuance.

Shipping:

Product should be shipped back to Covidien in such a manner as to prevent damage. Product damaged in shipment will not receive credit. The merchandise return authorization number must be clearly labeled on the shipping container. Product should be shipped to the address provided by Customer Service at the time that the return authorization is provided.

Freight:

Authorized merchandise returns are shipped at the customer's expense. Covidien does not allow for collect shipments on merchandise returns.

C.O.D. packages will not be accepted.

Credit:

Covidien does not provide credit for and will not return the following products:

  • Custom manufactured products (including kits).
  • Products not listed in the current price list or otherwise discontinued.
  • Products that are not in the original Covidien Packaging in our standard selling unit of measure (i.e.: full, unopened boxes).
  • Products that do not meet Covidien's standard for resale.
  • Products with less than six months shelf life remaining.
  • Any additional products not authorized on original Return Authorization.

Returned products that, upon receipt by Covidien, are damaged or otherwise not in conformance with the requirements of this returned goods policy shall be subject to disposition by Covidien in it's sole discretion, without credit or product exchange or return to the customer.

Credit for Authorized returns will be based on invoiced price.

Reason for Return Credit Issued

  • Defective Products Invoiced Price
  • Covidien Shipping Error Invoiced Price (with P.O. and within 90 days)
  • Covidien Order Error Invoiced Price (with P.O. and within 90 Days)
  • Inventory Balancing (with P.O.) Invoiced Price Less 20%

Customer debits in advance of the Covidien credit will be considered unauthorized .

Reusable Instruments

On loan, refurbished or returned products must be cleaned and decontaminated prior to return. A form will accompany your return authorization that requires your signature.
